Based in Romney the HFM will provide mentoring & case management services to customers in the Potomac Highlands Permanent Supportive Housing program.
Essential Duties/Responsibilities:
Place homeless, disabled or other targeted populations into stable housing, linking them with services to assist them to increase self-sufficiency and transition to permanent housing, reducing their risk of returning to homelessness.
1. Interview clients, assessing their situations, capabilities, and problems to determine what services are required to meet their needs and document eligibility for program admission.
2. Counsel clients regarding issues including mental health, poverty, unemployment, substance abuse, physical abuse, rehabilitation, social adjustment, child care, or medical care.
3. Refer clients to community resources for services, such as job placement, debt counseling, legal aid, housing, medical treatment, transportation or financial assistance, and provide concrete information, such as where to go and how to apply.
4. Develop and review goal plans in consultation with clients and perform follow-ups assessing the quantity and quality of services provided.
5. Maintain confidential case history records, ensure tracking of customer information and produce applicable reports.
6. Serve as liaisons between clients and other contacts, modeling appropriate problem-solving skills. De-escalate and mediate resident conflicts.
7. Where applicable, conducts regular housing group meetings with residents of the assigned program(s) to foster group self-governance at the housing site and to resolve problems or issues that arise.
8. Maintains office schedule and drop-in visitation/mentoring support to assist customers with information and guidance that advance goal attainment progress and overcome barriers.
9. Enforce applicable program and housing rules and regulations.
10. Work in coordination with Housing Counselor to assist clients with budget management and credit counseling issues to assist them in reaching financial independence and permanent housing.
11. Where applicable, locate and lease apartments that meet HUD quality standards, maintain yearly leases. Complete regular unit inspections and refer for maintenance if needed. Provision apartments at move-in.
12. Participate in community-wide meetings addressing issues and barriers to homelessness.
13. Actively engages in staff meetings and conference calls.
14. Participate in professional development activities.
15. Fill in during other staff absence.
